Incidents

Where: Safety Management › Incidents

Use Incidents to report anything that went wrong or nearly went wrong at work — a near miss, an unsafe act or condition, property damage, a fire, an environmental spill, a security or vehicle incident — and to follow it up until it is closed.

Incidents come from two places:

  • People — anyone with access raises one with Raise Incident, from a desktop or a phone. Attach a photo, or just describe what happened, and AI fills in most of the form.
  • Cameras — if your sites have connected cameras, hazards they detect (for example, a missing helmet) appear here automatically, with the camera's image.

How an incident moves​

Open  →  In Progress  →  Closed
\__________________→ Abandoned (raised by mistake, duplicate, not an incident)
  • A new incident starts as Open.
  • Whoever is following it up moves it to In Progress, and to Closed when it is resolved — each time with a comment and, ideally, an evidence photo.
  • If your organization has set up an incident workflow, you'll see its own stage names instead (for example Reported → Under Investigation → Actions in Progress → Closed), and some moves may need approval.
  • Open incidents are re-notified to the responsible people until someone acts on them.

Raise an incident​

  1. Open Safety Management › Incidents and click Raise Incident (top right).
  2. Take or attach a photo, or type what happened under Describe what happened.
  3. Click Analyze with AI and wait a few seconds. (Or Skip AI Analysis — Enter Manually.)
  4. Report Details: check the site, zone, incident type, date and time it happened, hazards and the people responsible. Fix anything AI got wrong, tick the confirmation box, click Next.
  5. Injury / Illness: answer whether anyone was hurt. If yes, fill in a block for each injured person. Tick the box, click Next.
  6. If you have access to Review & Investigation, check the Assessment and Recommended Actions steps the same way.
  7. Click Save. The incident appears in the list as Open and the people concerned are notified.

Follow up and close an incident​

  1. Click the incident in the list to open Incident Details.
  2. Read the media, info and comments.
  3. Click Update Status, choose the new status, write what was done in Enter Comments, attach an evidence photo, and Save.
  4. Repeat until the incident is Closed.

Update an action assigned to you​

  1. Open the incident (from the list, or the Open button in your notification email).
  2. Click View AI Analysis & Recommended Actions.
  3. Click the status chip next to your action (e.g. Open), set its Status (In Progress, then Complete), add a comment and evidence photo, and Save.

Record an injury after the incident was raised​

Open the incident and click Log Injury Case under Quick Actions. The injury case is linked to the incident and also appears in Injury Cases.

Find incidents​

Use the filters at the top of the dashboard — Site, Zone, Incident Source (Manual Incidents for people-raised ones, or a camera), Incident Severity and Duration — and the status tabs above the list. See Incidents dashboard.

Common questions​

I attached a photo but AI got the hazards wrong. Edit them on the Report Details step before you save — AI only drafts the form, you decide what is saved.

Can I raise an incident without a photo? Yes. Describe what happened and AI works from the text, or skip AI and fill in the form yourself.

Why can't I see the Assessment and Recommended Actions steps? They are only shown to users with access to Review & Investigation. AI still saves its analysis with the incident for the reviewers.

Why don't I see Site or Zone? Your organization has one site (or the site has one zone), so they are filled in for you.

What is "My Queue"? For team members, when an incident workflow is set up: the incidents waiting at a stage your team is responsible for.

What does SLA Adherence mean? With a workflow that has time limits per stage, it shows whether an incident is within its stage's time limit, close to it, or past it.
